Penile bruising can happen after medical procedures in the genital area, such as surgery on the penis, groin, scrotum, or surrounding areas, says Dr. Brahmbhatt. Hernia repairs, vasectomies, incision of infected abscesses and scrotal explorations, which are done to examine painful testicles, are common examples.

Penile bruising can extend into the scrotum and vice versa, says Dr. Brahmbhatt. “There is a plane of tissue that connects both,” he says. “If this layer is interrupted, then the bleeding can track from one to the other.”

Bruising is also common side effect of intralesional injection therapy, a treatment for Peyronie’s Disease, a condition in which penile scar tissue, known as plaques, give the member a curvature. Injections are sometimes used to break these plaques.
